The patient is 67 years old male complaining of gum recession and bleeding on brushing.

Medical history:

The patient is in good systemic health but smoking 5 cigarettes per day for the last 10 years.

Dental history:

The patient is missing all wisdom teeth and 17 was extracted due to deep caries lesion and fracture.

Intraoral exam:

Periodontal chart shows average probing depth (PD) of 6-7mm on majority of his teeth, and attachment loss (CAL) of 5mm. The greatest bone loss of 65% is detected on 16 mesial. Horizontal furcation involvement of class II is evident on mesial of tooth 16 and on lingual of 37.
